The Registration fee is $10.00 per player. This provide you with a player ID card and the ability to create a team and register for the EDDOA Futsal Series .

Coaches and Players MUST be registered through U.S Futsal to play:

U.S. Futsal utilizes Gotsport:

We offer multiple ways to pay, once your team has been registered, you will receive an invoice with the due amount. If you signed up for multiple teams/events, the price on the invoice will reflect the discount.
You can submit payment through:
- Venmo: @eddoafutsal
- Paypal: paypal.me/eddoafutsal
- Zelle: admin@eddoafutsal.org
- Gotsport: If paying through Gotsport make sure to get the discount code and enter it on the checkout page.

Rosters

Rosters need to be entered in Gotsport and approved before game day.
Please keep in mind, the referee will check in all players on Game 1.
Players can NOT play in BOTH divisions on the same night.
- Make sure your players are playing in one single division.
Girls are allowed to play within 1 year of their respective age.
- Example: 14G can play 15B.

Required Waivers

Each coach and player MUST sign 2 waivers:
- EDDOA Futsal Series – Waiver of Liability and Release Form
- UHCL Waiver
If an “Individual Player Waiver” wasn’t signed by your parent, your child will NOT show up on the gameday roster. So please make sure every player attending signs the waiver form.
